I understand it's fake, but is it "only for looking cool in a display case" fake? Or can i actually attach this to a handgun and fire it? Just without any actual noise suppression.

Gemtech says it "a display item and is not intended to be shot."  If it is hollow and open all the way through, I suppose shooting with it attached would be up to you.  But if things went bad, it would all be on you.

HydCVh, this is "Not to be Fired" since it is essentially a "Shell" and does not have the strength characteristics as the genuine article.
