Not reported David as I have seen or been aware of. The manual safety is in a peculiar place but, not reported issues with people who are or have become familiar with this setup.

No issues that I have heard of but like any gun you carry its up to you to KNOW the safe handling of the gun including how to use the safety

S&W has used this safety on their M&P pistols for quite some time, should be problem free.
