Aaron, the transfer form is ATF Form 4473 but you should not have to provide that, an FFL should have all necessary forms for you to fill out. You should just need your ID, CCW if you have it - only strictly necessary for handguns but it can be used to fill out paperwork for a rifle/shotgun as well, and a form of payment for the transfer fee.

Aaron, if they are not registered with Buds, just have them email or fax their FFL info to FFL@BudsGunShop.com - Fax : 1-800-804-5569 . If they are already registered, they will provide all transfer paperwork including Form 4473. You simply provide ID and pay their transfer fee $25 to $40 dollars.
