This is pretty much unheard of and sounds incredibly dangerous. I recommend contacting S&W customer service HERE ( https://www.smith-wesson.com/customer-service/contact-us ) I hope this helps!
David, this is definitely an assembly issue or defect with how far the slide flew. Hope no one was hurt/injured. Please contact Smith and Wesson Customer Service for advice/resolution at 1-800-331-0852 Ext. 2904 8:00 a.m. to 4:45 p.m. Eastern Standard Time, Monday - Friday
David, we're just customers as yourself not buds employees but after the firearm is transferred you would have to go through the manufacturer. You can contact Smith and Wesson at 1-800-331-0852 Monday-Friday 8:00AM-4:45PM, or you can email them at ( qa@smith-wesson.com ). Sorry you're having issues but hope this helps.
