Terry, the dealer that you choose for the transfer will guide you through the "Form 4." The transfer for NFA items happens at your dealer, and they'll make sure you have everything you need before they send the paperwork in. They might also have suggestions about setting up an NFA trust, etc. I'd find a class III or NFA dealer in your area to answer your questions as NFA regulations sometimes vary state to state.
