Hi Tracy, yes it does come with two cylinders (1) 22 Magnum and (1) 22LR as a combo. This is because of the difference in both case diameter and length of the 22 magnum and 22lr cartridges. The 22mag case is both longer and slightly wider diameter than the 22 long rifle (22lr) cartridge.

Yes, 22 mag requires a separate cylinder so it comes with one for it and one for 22lr. Manual states this as well. I have a bird's head grip version... shoots pretty well. Good luck.
