Rebate information can be found here: budsgunshop.com/rebates.php There is nothing to fill out during the purchase process as this rebate would be handled afterwards entirely though CZ using your proof of purchase.

Hi Charles, the rebate is from the manufacturer, so you don't add the rebate when ordering. You apply for your rebate from CZ after you receive your gun. Rebate form here >>> https://cz-usa-rebates.com/13267/
